Shelter, transitional and permanent supportive housing.

Services Provided
Housing opportunities include shelter, scattered and single site supportive housing. Shelter services are only accessible through 211 Infoline. Case management services include, recovery support/relapse prevention/harm reduction, employment services, and peer support. Street outreach offers support for people living outside, under bridges, in encampments, in abandoned buildings, providing basic needs and services to help people move toward housing and recovery.

Independence Northwest is the independent living resource for people with disabilities living in northwestern Connecticut. We work with people with all types of disabilities in all age groups. Independence Northwest operates the Housing Opportunities Program for individuals and families living with HIV/AIDS, with 35 scattered site subsidies - in the Greater Waterbury/Upper Naugatuck Valley and in Litchfield County.

Services Provided
The program offers advocacy, case management, emergency assistance funds, independent living skills training, peer counseling, assistive technology services, peer support counseling for substance abuse and mental health and referrals for permanent housing, substance abuse and mental health treatment, medical care, parenting/children’s needs, legal services and vocational training/rehabilitation.

Population Served
Independence Northwest offers a broad range of services for people with all disabilities. The Housing Opportunities program serves individuals and families living with HIV/AIDS from the Greater Waterbury, Upper Naugatuck Valley and Litchfield County regions of Connecticut.

Leeway, Inc. is a 30 bed skilled nursing facility and a 30 unit residential care housing program located in New Haven.

Services Provided
Leeway’s Skilled Nursing Facility provides compassionate, state of the art short term, long term and respite skilled nursing care to people living with HIV/AIDS. Services include IV therapy, wound care, physical, occupational and speech therapy, case management, mental health and substance abuse counseling, support groups and recreational activities. Leeway’s Residential Care Housing Program provides housing with 24-hour med-certified compassionate staff. Services include 3 meals a day, onsite laundry, access to Leeway’s Social Services Department with case management, mental health and substance abuse counseling, discharge planning services and recreational activities.

Population Served
Leeway’s Skilled Nursing Facility-Anyone who is 16 years of age or older, who has HIV/AIDS, and who needs 24-hour skilled nursing services is eligible for admission. Leeway’s Residential Care Housing Program-An individual must be age 65 and older; or disabled between the age of 18 and 65; or receiving SSI and/or SSD entitlements; or blind.

Liberty Community Services, Inc. operates a 16 bed transitional living program, a 7 bed permanent supportive living program, a 25 bed independent scattered site program, a 22 bed Safe Haven, a collaborative 26 bed intensive service scattered site program, and housing access and referral resource program.

Services Provided
Liberty Community Services offers a wide range of programs for homeless or at risk of becoming homeless adults living with HIV/AIDS. These programs include the Transitional Living Program, the Supportive Living Program, the Independent Living Program, Safe Haven and The Open Door Alliance (a PILOTS Initiative). Services provided by these programs include case management, counseling, addiction and recovery support, independent living skills development and access to rental assistance.

Liberty Community Services offers a variety of programs targeted at a multiplicity of special needs populations, including: Homeless or at risk of homelessness adults (all programs); Adults living with HIV/AIDS; Substance addicted; and Mentally ill.

New Reach, Inc. is a multi-service agency providing a continuum of housing and stabilization services to families in need since 1990. The SHP AIDS program operates 28 units of supportive housing to homeless or at risk families with HIV/AIDS. Six of the units are part of a New Reach owned supportive housing property, while the remaining 22 are scattered throughout New Haven and the surrounding towns. Families consists of any adult(s) with a dependent minor under the age of 18 living in the home. Services include intensive in-home case management services, clinical services (substance abuse / mental health), food, clothing, furniture and evening and weekend phone coverage.

Services Provided
New Reach offers case management services, advocacy, substance abuse/mental health referrals, support groups, housing location, and landlord/tenant mediation.

Population Served
The scattered site program serves families living with HIV/AIDS. The primary resident must be living with symptomatic HIV disease or an AIDS diagnosis and have a household income under 80% AMI. The agency serves families from the Greater New Haven area.
